How did you hear about us?

Upon approval of the application, a non-refundable $500 deposit is required to reserve a puppy. The deposit will be applied to the final adoption price of the puppy. If no puppies are available when the application is approved, the applicant will be placed on a waitlist and no deposit is required until a puppy is available. By sending this electronically, I acknowledge that I have read, comprehended and completed this questionnaire in its entirety. I certify that the above information is correct. I understand that the information will be verified by Skyland Golden Retrievers and its owners. I agree that Skyland Golden Retrievers and its owners can contact the applicant, references provided by the applicant and do any necessary investigations that will approve the application. I understand that upon approval of this application, further contracts and agreements will need to be read, comprehended, and signed in order to complete the adoption of a puppy.
